Kazanjian Vestibuloplasty to Improve Soft Tissue Conditions

This case demonstrates how a Kazanjian vestibuloplasty can restore peri-implant soft tissue quality and support prosthetic success. Our patient, a 50-year-old ex-smoker on Eutirox for hyperthyroidism, requested fixed prosthetic rehabilitation after years of edentulism.
Diagnosing the Problem: Gingival and Buccal Bone Loss
On examination, we found insufficient attached gingiva and resorption of the buccal bone. These conditions increase the risk of food impaction, inflammation, and implant failure.
To address both functional and biological needs, we proposed a single intervention combining implant placement with a Kazanjian vestibuloplasty.
Kazanjian Vestibuloplasty and Implant Placement
We used the Kazanjian technique to deepen the vestibule and increase the band of attached mucosa. This approach provides long-term protection for the implant necks while also improving access for hygiene.
During the same procedure, we placed two implants in the anterior mandible. The vestibuloplasty not only created a stable soft tissue seal but also encouraged new buccal bone formation by repositioning muscle insertions.
This integrated method allowed us to meet multiple objectives in one surgery.
Follow-Up and Results
At the one-month follow-up, we observed stable implants, keratinized mucosa, and a well-formed vestibule. These outcomes reduce the risk of peri-implantitis and contribute to long-term prosthetic stability.
