Vestibuloplasty and Implant in One Surgery: A Modern Approach

We’re presenting a technique that combines vestibuloplasty and implant in one surgery. This approach allows clinicians to address soft tissue and implant placement in a single step, improving outcomes and patient comfort.
Revisiting the Kazanjian Vestibuloplasty
Kazanjian vestibuloplasty was developed over 100 years ago by Dr. Varaztad Kazanjian. The method was intended for edentulous patients with mobile full dentures. It involved removing the muscular layer between the mucosa and periosteum to gain vestibular depth.
Today, it’s regaining relevance in implant surgery, especially when soft tissue conditions are poor.

Clinical Case: Soft Tissue and Implant in One Step
This patient came to us with an implant in position 36. To complete a bridge over 34–35–36, we needed to place an implant in position 34.
Soft tissue quality was insufficient. Instead of a conventional crestal approach, we performed a modified Kazanjian vestibuloplasty. This allowed us to gain keratinized gum and vestibular depth while placing the implant.
Benefits of One-Stage Vestibuloplasty and Implant Placement
This single surgical approach enabled us to place the implant and improve soft tissue conditions simultaneously—avoiding future surgeries.
