This case presents the management of a **post-extraction implant** in the anterior region, combined with a connective tissue graft to maintain volume and esthetics. The patient, a healthy 30-year-old non-smoker, was referred with root remains in tooth 22.

### From Extraction to Immediate Implant Placement

We performed an atraumatic extraction to preserve the alveolar bone and soft tissue profile. Immediately afterward, we placed the implant, aligning it with adjacent teeth and positioning it to emerge through the cingulum, which is ideal for esthetic restorations.

However, the buccal bone plate showed signs of collapse, a common situation in anterior extractions. If left untreated, this would compromise both esthetics and tissue stability.

### Why We Added a Connective Tissue Graft

To address the risk of soft tissue recession, we included a palatal **connective tissue graft**. We harvested it with a single-incision technique and inserted it via a tunnel through the gingival sulcus. This approach preserved the periosteal blood supply and eliminated the need for flap elevation.

As a result, the soft tissue gained volume and better long-term predictability. This strategy is especially useful in cases where esthetics are critical.

### Results and Case Evolution

The combination of a **post-extraction implant** and soft tissue enhancement produced excellent initial results. We’ll continue follow-up to assess mucosal stability and implant integration.
